“我是被推荐人的专业课导师并且熟识了四年” I am well acquanted with the applicant for over four years as his professional course teacher.
如果是单位领导的话,The applicant performs very well and we recommend him for abroad student。
“作为专业课导师，认识某人四年”可以翻译成“as XXX's professional course tutor,i have got acquaintanced with him(or her) for four years”
How long and in what capacity have you known the applicant?”这句话的意思是“您和申请人认识有多长时间，在何种场合认识？”
